Bring any forms with you that need to be completed. Some common forms are available below. Read and follow all form requirements thoroughly. In some cases:
- A urine test is required.
- You should be prepared to list all current medications.
- If using a CPAP or BiPAP, a 90-Day Summary Report is required at the time of CDL/DOT exams.
